What Is It?

Home Health care includes part-time or intermittent Skilled Nursing care, as well as other skilled services, such as Physical, Occupational, and Speech-Language Therapy. Services may also include Medical Social Services, and assistance from a Home Health Aide in conjunction with your skilled care.

Why Use It?

Healthcare professionals from MedSide work with you and your doctor to evaluate your health care needs and develop your plan of care. The plan of care tells you what Home Health services you need. MedSide will provide you with all the Home Health care listed in your plan of care. MedSide staff will teach you (or your authorized representative) how to maintain your independence at home.

What Are Its Goals?

Home Health care helps individuals, particularly seniors and those with disabilities, live independently for as long as possible. In general, the goal of Home Health care is to provide treatment for an illness or injury; to restore an individual back to their initial level of functioning; and to prevent hospitalizations. It helps patients get better, regain their independence, and become as self-sufficient as possible. If you have long-term health problems, the goal of Home Health care is to maintain your highest level of ability or health, and help you learn to manage your illness or injury.

  • Caring for hair, nail, foot, ear, oral hygiene, and skin care with non-medicated lotion or powder
  • Changing bed linens of an incontinent patient
  • Assistance with feeding and elimination (including routine catheter and colostomy care)
  • Assistance with ambulation, transfers and simple exercises, changing position in bed
  • Assist with medication reminders and perform simple dressing changes
